Abstract: Abstract of thesis entitled Suicidal Ideation in Patients with Early Psychosis Submitted by Heidi Chan for the degree of Master of Philosophy at The University of Hong Kong in August 2003 The present study aimed to study suicidal ideation in early psychosis patients. The clinical and psychological profiles of patients with and without suicidal ideation were comprehensively explored. Also, the possible relationships between the disorder, risk factors, and suicidal ideation were investigated. Eighty-nine in and out patients with early psychosis, aged between 15 to 27, were recruited from an early intervention programme. A set of comprehensive ratings was used to assess the clinical and psychological profiles in these patients: Positive and Negative Syndrome Scale (PANSS), Scale of Assessment of Negative Symptoms (SANS), Montogomery and Asberg Depression Rating Scale (MADRS), Simpson and Angus Scale, Abnormal Involuntary Movement Scale (AIMS), Barnes Akathisia Rating Scale (BARNES), Beck Scale of Suicide Ideation (BSS), Beck Hopelessness Scale, Levenson's Internal, Powerful Others and Chance Control Scale (IPC), Brief Reasons for living for adolescents (BRFL-A), Barratt Impulsiveness Scale (BIS-11), Life Event Questionnaire, Future Outlook Inventory, Drug Attitude Inventory (DAI) and Treatment Satisfaction Questionnaire. Forty two percent of patients reported current suicidal ideation. Patients with suicide ideation showed significantly less severe symptoms (p=0.001). Compared to non-ideators, patients with suicidal ideaton showed significantly more hopelessness (pWhen compared with 71 matched normal controls, it was found that suicidal ideation in patients and controls shared some common psychological factors (e.g. hopelessness, negative future outlook, and negative survival and coping beliefs), while the disorder itself may have intensified these common psychological risk factors of suicide among patients. These findings suggested that there were differential clinical and psychological profiles between suicide ideators and non-ideators among early psychosis patients. Suicidal ideation in patients with early psychosis shared some psychological risk factors with normal controls, while they also had some specific risk factors. Also, It suggested a possible role of psychosis in causing the excessive suicidal ideation, through negative effect of clinical factors and treatment-related factors, additional psychological risk factors, and intensification of mediating psychological risk factors of suicidal ideation.
