D.J. Betteridge is Professor of Endocrinology and Metabolism at University College London and Honorary Consultant Physician at UCL Hospitals NHS Trust, The Middlesex Hospital, London, UK. J.M. Morrell is both a family doctor and a hospital practitioner with an interest in CHD prevention.
